First Appointments
Kindly arrive at least 10 minutes prior to your first appointment to provide health information and consult with the practitioner regarding your condition. Contact our office for more information.
Illness and Health Concerns
Kindly let us know of any health conditions, allergies and/or other concerns you may have prior to treatment. In an effort to provide a healthy environment, we respectfully request that you reschedule your appointment if you are experiencing a cold, fever, or flu, etc. This is in consideration of your health as well as the well-being of other clients and staff.
